Counterfactual reasoning answers "What would have happened if…" questions by simulating alternative scenarios. It requires a structural causal model that specifies how variables interact. Counterfactuals are the highest rung of Pearl's Ladder of Causation (Association → Intervention → Counterfactual). Applications: explainable AI (why this decision?), fairness (would outcome change if gender changed?), medical diagnosis (what if treatment was different?).
